Grad Night Registration Form

Congratulations! You have taken the first step to a night of fun! Payment can be made here by credit card or you can pay by check/cash. In either case, please print the Registration Form and the Senior Release form (& the Emergency Care if applicable). The Senior Release form must be signed by both student & Parent even if the student is 18yrs old. Scan/email the forms to sge@hazenboosters.org or drop off in the Booster Club mailbox in the main office.

Grad Night will take place on June 11, 2025. Buses will leave Hazen at 10:00 PM. A non-refundable deposit or full payment is due at time of registration. A $50 cancellation fee applies.
